The pathogenesis of inflammatory bowel disease (IBD) is multifactorial with data suggesting the role of a disturbed interaction between the gut and the intestinal microbiota. A defective mucosal barrier may result in increased intestinal permeability which promotes the exposition to luminal content and triggers an immunological response that promotes intestinal inflammation. IBD patients display several defects in the many specialized components of mucosal barrier, from the mucus layer composition to the adhesion molecules that regulate paracellular permeability. These alterations may represent a primary dysfunction in Crohn’s disease, but they may also perpetuate chronic mucosal inflammation in ulcerative colitis. In clinical practice, several studies have documented that changes in intestinal permeability can predict IBD course. Functional tests, such as the sugar absorption tests or the novel imaging technique using confocal laser endomicroscopy, allow an in vivo assessment of gut barrier integrity. Antitumor necrosis factor-? (TNF-?) therapy reduces mucosal inflammation and restores intestinal permeability in IBD patients. Butyrate, zinc, and some probiotics also ameliorate mucosal barrier dysfunction but their use is still limited and further studies are needed before considering permeability manipulation as a therapeutic target in IBD. |

염증성 장질환 (IBD)의 발병 기전은 장과 장내 미생물 사이의 상호작용을방해하는 역할을 암시하는 자료로서 다수동의인자이다. 손상된 점막 장벽은 장내 투과성을 증가시켜 장내분자량이 큰 내용물과 유해물질들이 혈관으로 유입되어서 장의 염증을 촉진하는 면역반응을 유발할 수 있다. IBD 환자는점액 층 구성부터 세포 간 투과성을 조절하는 접착 분자에 이르기까지 점막 장벽의 여러 특수 구성 요소에서 몇 가지 결함을 보인다. 이러한 변화는 크론병 (Crohn’s disease)의 1차 기능 장애를 나타낼 수 있지만, 궤양성 대장염 (ulcerative colitis)의 만성 점막 염증을 지속시킬 수 있다. 임상실습에서 장 투과성의 변화가 IBD의 경과를 예측할 수 있다는 여러 연구가 발표되었다. 그러므로 당 흡수 테스트 또는 공유된 초점 레이저 내시경 (confocallaser endomicroscopy)을 이용한 새로운 영상 기술과 같은 기능 테스트를 통해 장벽(腸壁)의 온전성에 대한 생체 내 평가가 가능하게 되었다. 항 종양 괴사인자-α (TNF-α) 요법은 점막 염증을 감소시키고 IBD 환자의장 투과성을 회복시킨다. 뷰티레이트 (butyrate), 아연 및 일부프로바이오틱스는 점막 장벽 기능 장애를 개선하지만 그 사용이 여전히 제한적이므로, IBD의 치료 목표로써장 투과성의 조절을 고려하기 이전에 추가 연구가 필요하다.
