The effectsof Lactobacillus plantarum on small intestinal barrier function and mucosalgene transcription; a randomized double-blind placebo-controlled trial

The aim of this study was to investigate the effects of three Lactobacillus plantarum strains on in-vivo small intestinal barrier function and gut mucosal gene transcription in human subjects. The strains were selected for their differential effects on TLR signaling and tight junction protein rearrangement, which may lead to beneficial effects in a stressed human gut mucosa. Ten healthy volunteers participated in four different intervention periods: 7-day oral intake of either L. plantarum WCFS1, CIP104448, TIFN101 or placebo, proceeded by a 4 weeks wash-out period. Lactulose-rhamnose ratio (an indicator of small intestinal permeability) increased after intake of indomethacin, which was given as an artificial stressor of the gut mucosal barrier (mean ratio 0.06 ± 0.04 to 0.10 ± 0.06, p = 0.001), but was not significantly affected by the bacterial interventions. However, analysis in small intestinal biopsies, obtained by gastroduodenoscopy, demonstrated that particularly L. plantarum TIFN101 modulated gene transcription pathways related to cell-cell adhesion with high turnover of genes involved in tight and adhesion junction protein synthesis and degradation (e.g. actinin alpha-4, metalloproteinase-2). These effects were less pronounced for L. plantarum WCFS1 and CIP104448. In conclusion, L. plantarum TIFN101 induced the most pronounced probiotic properties with specific gene transcriptional effects on repair processes in the compromised intestine of healthy subjects. |

본 연구의 목적은 인간 피험자에서 생체 내 소장 장벽 기능 및 장 점막 유전자 전사(gene transcription)에 미치는 세 가지 락토바실러스 플란타럼(Lactobacillus plantarum) 균주의 영향을 조사하는것이었다. 이 균주는 TLR 신호 전달 및 밀착 결합 단백질재배치(tight junction protein rearrangement)에 대한 차별적 효과들을 위해선택되었으며, 이는 스트레스를 받은 사람의 장 점막에서 유익한 효과를 이끌어 낼 수 있다. 10명의 건강한 지원자가 4가지 다른 개입 기간에 참여했다. 4주간의 세척(wash-out) 기간이 선행된 후, 각각 L.plantarum WCFS1, CIP104448, TIFN101 또는 위약 중 하나를 7일간 경구 섭취했다. 락툴로오스-람노오스비율 (Lactulose-rhamnose ratio; 소장 투과율 지표)는 인도메타신(Indomethacin)을 섭취한 후 증가하였는데 이는장벽 점막의 인위적 스트레스로써 주어진 것이었으나 (평균 비율0.06 ± 0.04에서 0.10 ± 0.06, p = 0.001), 균 개입에 의한 유의한영향을 받지는 않았다. 그러나 위십이지장경검사(gastroduodenoscopy)에서얻은 소장 생채 검사 분석 결과는 특히 L.plantarum TIFN101이 밀착 및 접착 결합 단백질 합성 및 분해에 관여하는유전자의 높은 전환율(예: actinin alpha-4,metalloproteinase-2)을 보이는 세포 간 접착과 관련된 유전자 전사를 조정한다고 서술한다. 이러한 효과는 L. plantarum WCFS1 및 CIP104448에서는 덜 두드러졌다. 결론적으로, L. plantarum TIFN101은건강한 피험자의 면역 반응 등의 기능이 제대로 되지 않는 내장(compromised intestine)의복구 과정에 특수한 유전자 전사 효과를 갖는 가장 두드러진 프로바이오틱 특성을 초래했다.
